STEP 1


Pre-Registration Form



Complete the Pre-Registration Form to register your group for CityConnexx 2008. Your deposit will lock in your group rate based on an estimate of students attending and will provide Connexx with important details for matching your group to a ministry site. Don't miss the deadlines listed in Section 4 on page 2 to lock in the best rate!

STEP 2


Final Registration Form



Once you've locked in your group rate with Step 1, complete the Final Registration Form and return to Connexx at YouthPartnersNET with your final payment 30 days before your arrival date.

STEP 3


CityConnexx Participant Medical Form



Complete one copy of the Medical Form for each individual attending CityConnexx (students and chaperones). Attach a copy of the individual's current health insurance card and bring them with you when you travel to your ministry site. These documents are required at the CityConnexx registration on your arrival date and will be collected by your Ministry Site Coordinator.
